To manage the disability database system and provide disability identification cards for use in social assistance programs and social services, in accordance with government policies and regulations.
There are 5 types of disability: physical disability, sensory disability, intellectual disability, mental/psychiatric disability, and other disabilities.
There are 3 levels of disability: Level 1 (Severe & Most Severe), Level 2 (Moderate), and Level 3 (Mild).
It is a database system capable of entering, storing, and sharing disability data for use in formulating policies, social assistance programs, and social services by public institutions, the private sector, civil society, and local communities.
Individuals of all ages, genders, and public officials with disabilities can undergo the disability identification process.
Persons with disabilities must present personal identification documents such as a Cambodian National ID Card, residency book, family book, birth certificate, or residency certificate; an equity card (IDPoor 1 or 2) if applicable; and a medical certificate or health status document for individuals with internal surgeries or chronic illnesses.
Persons with disabilities can undergo the interview at the commune/Sangkat office where they reside, or they can request the commune/Sangkat working group to conduct the interview at their home.
The designated official in charge of disability identification at the commune/Sangkat level.
Disability identification can be conducted during any official working hours of the designated officers.
It is conducted through direct interviews with the persons with disabilities, based on the questionnaire in the system, to verify and evaluate their condition through the system for disability identification.
After the interview for disability identification is completed, the person with disability receives a disability identification card.
Persons with disabilities can collect their disability identification card at the commune/Sangkat hall after being notified, or the village chief will deliver it directly to their home.
Persons with disabilities from households with an equity card can use the disability identification card to register for the national social assistance program in family packages to receive allowances.
The disability identification card for adult persons with disabilities is permanently valid. Children with disabilities under 8 years of age must update their data upon reaching 8 years of age to receive an adult disability identification card.
The cardholder should report the loss at the commune/Sangkat office to request the Ministry of Social Affairs, Veterans and Youth Rehabilitation (MoSVY) to issue a duplicate disability identification card.
The cardholder should report the issue at the commune/Sangkat office, bringing the old incorrect card, to request the Ministry of Social Affairs, Veterans and Youth Rehabilitation (MoSVY) to issue a new correct card.
All citizens or stakeholders can provide feedback orally to the Commune/Sangkat Working Group. Alternatively, you can enter them directly in writing via the Public DMIS App or the official website.
Everyone can participate, including local administrations (District/Commune), health center personnel, police, school teachers, and social affairs working groups.
It can take up to 4 weeks to receive official feedback, depending on the complexity and nature of the objection or suggestion provided.
If you are a registered data user and you forgot your password, you can reset your password automatically via email or you can contact the DMIS support team via support@mosvy.gov.kh.
Completed interviews cannot be deleted. However, you can file a request to edit household information by filling out form 2.3 and sending it to the Commune/Sangkat office or through the Public DMIS App.
Each request for an interview must be discussed by the Commune Sangkat Working Group (CSWG). In case the request was unintentionally entered, the CSWG can decide not to conduct the interview and document the reason.
If a household loses its Disability card, the Commune Sangkat Office must be informed. Options include: 1. Informing the office or PDOP verbally, 2. Filling out a complaint form, or 3. Filing a request via the DMIS App or website.
Please file a request to edit household information by filling out form 2.3 and submitting it to the Commune Sangkat or via the DMIS App. Local authorities, school directors, or health staff can also assist you.
Households can request an interview through form 2.1 at the Commune/Sangkat or apply via the DMIS App. Stakeholders like village chiefs and NGOs can also facilitate this interview request.
If the interview results show eligibility, you will receive the card from your village chief or the Commune Sangkat Working Group.
Eligible households typically receive their Disability card within 30-45 days after the date of the interview.
You can find the data query section on the DMIS websystem. It consists of dashboards showing aggregate information. Registered users can access more detailed data, filter by household characteristics, and export reports. For special requests, contact support@mosvy.gov.kh.
There are specific categories to which individuals are assigned based on questionnaire scores and special circumstances. These levels help determine the severity of disability and the corresponding support required.
Data reports for specific queries can be exported as Excel or PDF files using the download button in the data query section. You can choose the report type you wish to export.
Filters include disability level, gender, and administrative units. Registered users have access to advanced filter options. More information is available in the "Register as a Data User" section.
To register, provide contact details and the purpose of data use to the Ministry. Access levels vary based on the information provided and the user type assigned.
There are unregistered guest users (aggregate stats only) and three types of registered users: Basic, Standard, and Advanced. Registered users can access more data and track their requests.
